  • Write it down before you come

Write down any questions you may have prior to your visit.

  • Gather your information

Please bring your photo ID and current insurance card to each visit. We accept most insurance cards. However, there are some insurance policies with which we do not participate, which will cause a greater out-of-pocket expense to you. Please verify that we participate with your insurance policy by calling your insurance company directly. Please bring a list of the medicines you are taking to your visit, including names and doses. You also need to bring all of your medications in their original containers with you to ensure that the correct information is entered into your medical record. Please bring a copy of all medical records and discs that you have in your possession that are related to your visit, including any recent labs or EKG’s, heart catheterizations, echocardiograms, and stress tests. Please be prepared to provide a list of contact numbers of Providers that you have seen in the past for the related medical condition.

  • Bring someone with you

It's helpful to have an extra set of hands and ears when hearing new medical information, so if possible, bring another adult with you to the appointments. If we can help you in any way, please let us know.

  • Arrive early

Traffic and parking often take longer than you may expect. Please allow 10 to 15 minutes for the check-in process for an established patient and 25 to 30 minutes for a new patient. Delays and emergencies do happen, so be prepared.
If you have to wait for your doctor because of an unexpected emergency or on a very busy day, remember that you will receive the same immediate care and attention if you are in a similar situation.

  • Make the most of your appointment

Ask questions while meeting with your doctor. Also, remember to ask for any instructions from the doctor or letters of medical necessity that you may need. Please provide the nurse with any work forms that need to be completed. Remember to ask the nurse for a work or school excuse if applicable.

  • After the appointment

A copy of your visit summary will be provided to you at the end of your visit. Please review this document carefully and ask any questions that are not covered in the summary and discharge instructions. When possible, please allow the staff to schedule your follow-up appointment before you leave and write down new appointment dates and times. Also write down any other questions you have and important phone numbers to call, such as the clinic's nursing line, prescription refill line, and appointment line.
